It’s very important to us that you have a great experience with our doctor and staff. Your first visit establishes the foundation for our relationship with you, and we want to make sure it goes as smoothly as possible.
In order to do that, we need to obtain some important background information from you. The information on this page can help you prepare and understand what to expect before your first visit.
Patient Info Forms
In order to speed up the process at the front desk, minimize your stress, and give us as much time as possible to get to know each other, we encourage you to complete your patient paperwork before your appointment.
Patients are able to fill out and sign paperwork electronically. This can be done easily from your mobile phone, desktop, or tablet. Click on the link below to complete your new patient paperwork.
If you must complete your new patient forms in the office at your appointment, you must arrive 15-30 minutes early to do so. If you arrive at or after your appointment time with incomplete paperwork your appointment would need to be rescheduled due to the time it takes to complete the paperwork. If you are unable to complete the paperwork electronically, we can e-mail, fax, or mail a copy of the paperwork to you before your appointment. Feel free to stop by the office and pick up a copy if the new patient paperwork to complete and bring to your appointment.

Once completed, you may:
- Bring the completed paperwork with you to your appointment
- Fax your paperwork to our secure fax line at (972) 291-2533
- Mail your paperwork to Attn: Front Desk 1801 N. Hampton Rd. Suite 340, DeSoto, TX 75115. If choosing to mail documents, please allow adequate time before your appointment for the mail to arrive at our office.
If for any reason you cannot keep a scheduled appointment or will be delayed, please contact us as soon as possible so that we can offer your spot to another person in need of care.
What to Expect
During your appointment, we will have many questions for you—about your condition, your symptoms, and even your lifestyle (what you do for a living, your hobbies, etc.).
Although this may seem strange at first, your answers help tell us what we need to do. Our treatments are always tailored to the specific, individual needs of each patient. The more we know about you and your needs, the better we can provide treatments that will be effective at getting you back to your best with the minimum possible disruption to your lifestyle.
In order to relieve your stress, you may consider taking the following steps before your appointment:
- Writing down a list of medications you are taking
- Writing out a list of questions you have for us
- Reviewing the pages on this website to learn more about your symptoms or condition
